Da Vinci Surgical System For Robotic Surgery

OVERVIEW

More than 3 centuries ago, Leonardo daVinci was the first to devise a mechanical robot of a knight in armor to amuse the royalty. Intuitive Surgical has pioneered the robotic technique by applying it to minimally invasive surgeries, first with heart surgery, then with prostate surgery, and now it is available for women's surgery. Dr. McLaughlin has been a leader in the field of GYN robotic surgery by initially proctoring other physicians interested in learning this technique to offering a one-year postgraduate Fellowship in GYN Endoscopy.

Traditionally, Total Abdominal Hysterectomy (TAH) or Partial Hysterectomy (where the ovaries are preserved) required a 6-8 week recovery period. Patients undergoing Vaginal Hysterectomy (VH) had a reduced recuperation, but many patients weren't candidates for this procedure. Now, the hysteredtomy recovery time has been reduced with Laparoscopic Hysterectomy, either total or partial, but there were technical limitations due to the equipment available. With the daVinci robot system, many of these technical limitations have been eliminated, to the great benefit of our patients.

The time it takes to return to everyday life after major surgery has been markedly shortened with less postoperative discomfort through the da Vinci Surgical System. This new pelviscopy technique has revolutionized minimally invasive surgery to enhance current pelviscopy techniques for the patients' benefits. Dr. David McLaughlin, director of Women's Specialty Health Centers in Noblesville, performed the first gynecologic surgery using the da Vinci System in Indiana on July 8, 2005. Initially used to perform minimally invasive hysterectomy, Dr. McLaughlin has expanded its use for fertility procedures with a shortened recovery time and less postoperative pain. These hysterectomy alternatives include using the robot for removal of fibroid tumors (myomectomy), excision of ovarian cysts, treatment of endometriosis, ovarian tissue banking for fertility preservation and tubal reanastomosis for sterilization reversal.

Patients undergoing surgery with the da Vinci System usually only have a one-to-two-day overnight stay in the hospital, instead of three to five days for open abdominal surgery, with an expected return to normal function at work or home within one to two weeks. This revolutionary technique for gynecologic surgery is similar to the conversion from major gallbladder surgery to less invasive operative laparoscopy.

HOW IT WORKS

Through five dime-size incisions, Dr. McLaughlin inserts the da Vinci instruments into the abdomen. Through the central incision, a magnifying 3D optical system is placed to optimize visualization of the pelvic area. Through the other incisions, microsurgical instruments, which have the same range of motion as Dr. McLaughlin's hand, are inserted. Next to the patient, in the same operating room, Dr. McLaughlin sits at a remote console to precisely control the da Vinci instrument movements.

The procedure usually takes between two and three hours.
