What to bring with you at your new patient appointment
- Completed gynecology, insurance and registration forms
- Medical Records-If you wish to bring medical records from prior treatments, we have also provided a medical release form that can be downloaded and printed. It is recommended that you take the form to your doctor’s office when requesting copies of your records.
- Medical Insurance card
- Photo Identification
- List of current medications and dosages
- List of questions and concerns
- All fees are due at the time of service.
