During your initial appointment, our healthcare team will:

Review your medical history and current health concerns
Discuss your symptoms, goals, and questions
Perform any necessary examinations or evaluations
Recommend appropriate testing or screenings when indicated
Develop a personalized care plan designed specifically for you

We recommend that you allow yourself 2 hours for your first appointment. 

 

Our philosophy is to identify the underlying cause of your concerns whenever possible, allowing us to provide thoughtful, individualized treatment rather than simply treating symptoms.

We will send you your new patient documents and any required labs prior to your first appointment. We do ask that you complete all required documents prior to your appointment. Please bring the following:

 

Insurance card(s) - this will be needed for any prescriptions, labs, or imaging that may be ordered. 
Photo identification
Current medication list
Relevant medical records or test results, if available.
